Insurance & Payment
Before booking, please verify your benefits with your insurance company. This will help you understand your co-pay, co-insurance, and deductible. While I am in-network with several major carriers, some policies may not cover my services. I cannot guarantee your coverage, and you are responsible for any portion your insurance does not pay.

Please note: I am unable to see patients with Medicare or Medicaid/OHP plans (including Molina, Trillium, Healthsare, or CareOregon).
Out-of-Network or Self-Pay
If I am out-of-network with your insurance:
You may choose to pay out of pocket at the time of service.
I can provide a Superbill (itemized receipt) to submit to your insurance company for possible reimbursement.
Out-of-network coverage varies widely, so I encourage you to call your insurance provider for details.
I also welcome patients who prefer to pay privately or use HSA/FSA cards.
