NOTE: Before booking, please verify your benefits with your insurance company. This will help you understand your co-pay, co-insurance, and deductible. While I am in-network with several major carriers, some policies may not cover my services and I do not want you to have any surprise bills. If you do not check with your insurance company prior to your first appointment, you will be charged for any balance that is not covered by your plan.

I am currently in network with: Moda, Blue Cross Blue Shield (Regence, Anthem), Aetna, and Optum/United Healthcare

Please note: I am unable to see patients with Medicare or Medicaid/OHP plans (including Molina, Trillium, Healthshare, or CareOregon).

Out-of-Network or Self-Pay

If I am out-of-network with your insurance:

• You may choose to pay out of pocket at the time of service.

• I can provide a Superbill (itemized receipt) to submit to your insurance company for possible reimbursement.

• Out-of-network coverage varies widely, so I encourage you to call your insurance provider for details.

I also welcome patients who prefer to pay privately or use HSA/FSA cards.
